Once you have made a booking or have decided to have your tax return completed with Erik Smith, please select and complete one of the relevant forms below. You can then be added to the ATO Practice and have Erik Smith registered as your Tax Accountant/Agent. This is optional for consultations but is a requirement for the preparation and lodgment of your tax return. Please contact us if you have any questions or difficulties.
